Works Permits / High School Transcripts

 

 

(For SLVUSD Students ONLY)

 

Works permits are available through Charlotte Achen or Sara Sizemore at SLV High School during the regular school year. You may contact Charlotte Achen at (831) 335-4425, extension 215, or Jeannette Lange at (831) 335-4425, extension 216 or by email (above).

 

During the month of July, Work Permits will be available through Lisa Mansfield at the District Office. Please contact Lisa for instructions on how to obtain a work permit online, or to make an appointment. She can be reached by phone at 831-336-8852 or via email at [email protected].

 

2024 - The dates that work permits are available through the high school (above) are through June 24, 2024 and then again after July 22, 2024.   For June 25, 2024 through July 22, 2024 - Work Permits are available through Lisa Mansfield (above) at the District Office.

 

Work Permit Instructions:   STEP ONE:  Apply for and be offered a job. A Work Permit is required -after- this step and you must be at or above the age of 14 to work as a minor.   STEP TWO:  Print out, then complete the Work Permit and have both a parent and employer sign it.   STEP THREE:  Take the completed form to either the Counseling Office or District Office (depending on the date - see above).
