Student Health Forms
For students with medical concerns requiring medications to be administered at school and/or possible medical treatment provided by staff, it is required to have a school health care form on file that is signed by both the physician and the parent / guardian. After completion of the appropriate form, please bring the form(s) to the front office of the school site your child attends.
